it looks like part 904968 has the mount, isolator and washers for <$25.

I went with these. You really won't know until you pull it apart, I pulled it apart, lifted it, and had a popping noise from the front, ordered new struts and mounts, when I pulled it apart the 2nd time, I looked at the old mounts, the rubber had separated from the metal collar. I would either pull it apart and order what you need, or order the mounts, and if you don't need them, return them. It's a pain to do it all twice.

When I have the guy install the lift(if I get Marks) I will have him replace the Strut Mounts right then and there.
